To expedite the process of a duplicate of your lost CE Certificate, fill out all information below and process your electronic payment on our secure on-line server. We will then manually check the course, date and location for your signature on the sign in/sign out sheet. If this information is missing from the form below - we cannot process your request. Once we process your request we will bill your credit card the replacement cost of $35.00 per certificate.
